In that case, my only defense was a raised bridge blocking the main entrance to the fortress. Most of my dwarves were occupied clearing the great hall, and smoothing it out. Took a couple of years to actually build just that hall; IIRC. I had to cut off digging the drain out to the ocean because it would have left my fortress vulnerable. Fish bone vs Cube room design. Every other room type is similar. You can add doctors to a hospital the same as you can add performers and tavern keepers to the tavern.
